This includes all nurse practice acts such as assessment, medication management, documentation, supervision of other nursing personnel and performing as the care manager. Essential Job Functions Maintains and actively promotes effective communication with all individuals. Participate as part of the core team providing care and services and be designated to coordinate the implementation of the plan of care. Actively participate in Home Health & Hospice sponsored in-service training, and continuing education opportunities. Ability to adapt to change. Experience & Training Must be currently licensed as a Registered Nurse in the State of Wisconsin. At least 2 years of experience in Medical Surgical, Home Health, Hospice, or Long-Term care nursing preferred. Must be sensitive to the need of older adults. Certified in Gerontology, Medical Coding, Wound Care (WCC), Hospice and Palliative Nurse preferred. Schedule Monday to Friday: 8:00 AM to 4:30 PM 0.8 FTE (32 hours a week), 1.0 FTE (40 hours a week) On-Call Requirements This role includes on-call coverage, with additional incentives provided.
