Job Description This is a unique opportunity to be part of a tight-knit, collaborative team from across the health sector that will deliver the next generation of integrated care in Mississauga, providing holistic proactive, seamless care and support to patients, families and caregivers. In this role you will be responsible for conducting assessments, developing and delivering treatment plans focused on achieving the goals of care including mobility and function, symptom management, and delivering education.
