Responsibilities Contributes toward accomplishment of patient’s plan of care goals by providing mobility activities as assigned by the nurse. Assists patients and their families in learning to safely manage personal care while increasing activity, strength, and endurance. Effectively works with the patient and their interdisciplinary care team, reinforcing and promoting therapy/mobility initiatives. Effectively communicates changes in patient condition and functional ability to support an expedited change in the patient’s plan of care and prevention of more serious complications. Demonstrates the knowledge that patient and workplace safety is a priority by preventing errors, accidents and infections and reporting potential or actual concerns immediately. Utilizes a holistic approach (considering physical, psych/social, spiritual, educational, safety, and related criteria) appropriate to the age of the patients served in the assigned service area. Qualifications License(s)/Certification(s): High School Diploma or equivalent Other: Valid driver’s license when driving any vehicle for work-related reasons. Must possess and maintain current Basic Life Support (BLS) certification or obtain certification within three (3) months from date of hire. MOAB (will be obtained during training)
