CERTIFICATION/EXPERIENCE/KNOWLEDGE:

• Regulated Health Care Professional (e.g. Nursing, OT, SW, BT) in good standing with respective professional college required;
• Bachelor’s University Degree required;
• Certification in Gerontology/Mental Health Canadian Nursing Association an asset;
• Minimum of three (3) years of recent relevant clinical experience working with older adults experiencing mental health, dementia, delirium, substance use & other neurological diagnoses required;
• Certificate and applied skills in the following fundamental BSO core competencies: GPA, P.I.E.C.E.S., Dementiability and Recovery Philosophy required;
• Demonstrated knowledge and required skills to guide critical best practice elements for successful care transitions across sectors for older adults and their care partners required;
• Proficient knowledge of geriatric pharmacological best practice principles required;
• Demonstrated cultural awareness & safety in working with diverse older adults and care partners required;
• Demonstrated knowledge to practice facilitation skills in alignment with best practice adult learning principles required;
• Expert knowledge of Community Support Services, Primary Health Care and Mental Health System Resources & Partners and Consent & Capacity legislation (e.g. Public Guardian Trustee, Substitute Decision Maker Act, Health Care Consent Act, Mental Health Act, Long-Term Care Act, PHIPA) required;

Skills/Abilities:

• Demonstrated clinical competence, leadership and applied inter-collaborative skills to promote, facilitate and integrate best practice principles in holistic gerontological care including mental health, behavioural health, physical health and spiritual health required;
• Demonstrated excellence in clinical-reasoning, critical thinking skills, assessment, documentation, intervention and care planning skills required;
• Demonstrated quality professionalism in oral, written & interpersonal communication skills among regional care partners to convey essential clinical and systems information required;
• Commitment to health promotion & prevention, quality improvement and change initiatives in order to actively contribute to critical program evaluation and relevant research projects required; Lean Training/Certifications an asset;
• Ability to function collaboratively within an interdisciplinary team and skilfully prioritize competing workload responsibilities in a self-direct manner required;
• Progressive computer literacy skills, including adept electronic documentation skills and ability to navigate various electronic platforms (e.g. OTN) and software (e.g. Zoom etc.) required;
• Must support and contribute to a culture of safety and prevention of adverse events in this organization required;
• Must have a valid driver’s license, willingness to travel on occasion and access to use of reliable vehicle is required.
• Good attendance and work record required
• Bilingualism (Advanced level English/French) is required. All unilingual candidates are encouraged to apply and may be considered if the identified French requirement has been met or there are no successful bilingual candidates. A formal French test will be conducted to evaluate the proficiency level.

Criminal Reference Check including the Vulnerable Sector Check recent within six (6) months is a requirement

• Keeping abreast of new research and literature in the field of neurocognitive disorders, geriatric mental health, substance use and neurological conditions.
• Providing in the moment clinical coaching, system navigation to general public, community partners and BSO team members.
• Participating in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
• Determining and aligning improvement projects within North East BSO’s Regional Office; monitoring and adjusting to achieve goal outcomes.
• Representing the program on various committees and in meetings as required.
• May be required to assess clients in their home environment to determine eligibility and connections with the most appropriate services.
• As required, may carry a small caseload of individuals requiring behavioural support services, providing outreach visits, behavioural support assessments, engagement with consulting geriatric physicians as appropriate and connection with other services
