ABOUT THE ROLE:

The D2A (Discharge to Assess) service provides short-term, high-quality support to individuals who are well enough not to need an acute hospital bed but do need some extra support in their home. You will ….

• Write up support plans following the initial visit, ensuring person-centred care is provided
• Motivate customers, building their confidence so they can achieve their goals and return to independent living
• Support customers with tasks around the home and with personal care
• Work a mixture of early and late shifts between 7 am and 10 pm and alternate weekends.
• Be on-call on a roster basis which attracts an additional payment.
