About the Role
We are looking for a Life Skills Worker to join our multi-disciplinary team on a Full-time basis. This position requires working with the client in their home and community environment. Promotes the physical, mental, and emotional well-being of the individual. The worker will assist in goals as determined by the individual and their support team.

QUALIFICATIONS:

  • Social or Health Sciences Diploma and/or experience in related field
  • Able to communicate effectively with families, community partners and the KCDC team. Able to express self clearly, both verbally and in writing
  • Experience and understanding of the following skills: nutrition, general health and lifestyle issues, and developmental disabilities.
  • Knowledge of community resources for referrals

ADDITIONAL REQUIREMENTS:

  • A criminal record check
  • Current Class 5 Driver’s License
  • Available Monday to Friday and able to occasionally work flexible hours.

Responsibilities:

  • Establishes and maintains a trusting relationship with the client by providing regular contact as established by individual contracts with CLBC or Northern Health Authority
  • Under the direction of the Program Coordinator, implement the Program in accordance with the policies and philosophy of the Kitimat CDC
  • Participate as a team member in a family centered model of service; participate in team meetings and case conferences as appropriate
  • Refer and support the client in accessing appropriate community resources as necessary
  • Establish a schedule with client
  • Establish a relationship with the individual that allows for open discussion of issues and concerns
  • Establish an effective individual Support Plan as set out in the individual contract with CLBC and Northern Health Authority and help the client to reach goals set out

Administrative Duties

  • Maintain program records and statistical data required by the Centre and program funding
  • Participate as required in family service plan and staff meetings
  • To communicate verbally and in writing with other members of the Kitimat CDC team, public health, physicians, and other community service partners
