Description:

We are looking for full time Therapy Assistant Practitioner to join our dedicated Community Stroke Team across the South of Nottinghamshire. We are looking for someone is able to demonstrate motivation, drive commitment and passion for promoting health, well-being and independence for stroke patients in the community. We are a values-based Trust and are looking for dedicated and enthusiastic clinicians who believe in the provision of patient-centred care. As a Community Stroke service, we aim to offer a high quality therapy led Multidisciplinary Team approach providing stroke rehabilitation in a patients home focused around achievement of agreed rehabilitation goals.
As an Assistant Practitioner in the Community Stroke team you would be expected to undertake therapy assessments and devise treatment plans with indirect support of the qualified clinicians. We are an experienced existing provider of Community Stroke Rehabilitation and as part of our service we provide a review for patients after stroke at 6 and 12 months. As an Assistant Practitioner you would be required to undertake these assessments as part of this role when required. This encompasses completing a holistic review of all the service users health needs related to their stroke, together with advice and signposting for services.

The Community Stroke Team is split into three localities. The base of this role is within in the locality of Rushcliffe. The team work closely and will require you to work flexibly, across the whole of South Nottinghamshire, to support our evolving and developing service, supporting the team to deliver high quality integrated care. This position requires you to work independently, but also be a team player, working closely with our Integrated Care Provider partners providing care to a diverse population covering many different geographies including, Broxtowe, Gedling and Rushcliffe.
