An opportunity has opened up for a talented CPN to join the West End PCN Community Mental Health Service based in the heart of London , where you will be afforded the time and space to develop into a more enhanced and effective community practitioner.
We are searching for candidates from the following professions: Registered Mental Nurses who are looking for a new challenge.
West End PCN CMHT service - Monday to Friday 9am-5pm
Community Services are becoming fast-paced and rapidly changing to support the ‘Out of Hospital Care’ strategies. You will therefore need to demonstrate resilience and the ability to adapt to the changing landscape and opportunities.

Based in South Westminster, we are near Victoria Station with its vibrant, beautiful shops, eateries and Hyde Park where staff can take advantage of the more peaceful areas.
The successful applicant may have contact with patients or service users. As an NHS Trust we strongly encourage and support vaccination as this remains the best way to protect yourself, your family, your colleagues and of course patients and service users when working on our healthcare settings.
West End PCN CMHT – You will be expected to case manage an allocation of patients, monitor patients’ mental state, promote independence, promote social inclusion, aid the recovery process, and reduce hospital admissions.
You will also be expected to work in collaboration with the Operational Team Manager, Clinical Leads/Senior Practitioners and to have close working relationships with the community teams, inpatient services, and the HTT backup across the service.

Commitment from the team to develop you by offering coaching and role-specific training opportunities such as Leadership programmes , CBT , DBT and more
Opportunities to shadow our neighbouring services including CMHTs, HTTs, acute inpatients, PICU and HBPoS allowing you to see the full circle of treatment for our patients
Local Reflective Practice forums run by one of the senior leads
Clear objectives and pathway to develop your career further
CMHT service is Monday to Friday 9am till 5pm;
Each PCN is led by the same Consultant which allows for a clear understanding of each individual patient and allows for the ability to work collaboratively together to provide the best possible outcome for the patient.
