Description:
JOB SUMMARY
Reporting to the Director of Care, the Care Manager is responsible for leading the relevant inter-disciplinary care team members in the provision of care and services for residents based on the PPSL Fine Homes Care Model and residents’ choice. Coordinates and supervises the care team and provides guidance, direction and discipline as required to ensure the delivery of resident-centered care based on residents’ individualized care plans and PPSL policies and procedures. The Care Manager demonstrates knowledge of leadership and of mental health/gerontology principles and best practices in accordance with the applicable provincial registering body, any provincial or local health services mandate and PPSL policies and procedures. The Care Manager will ensure a commitment to resident safety and to our “culture of safety” as per PPSL Strategic Directions.
QUALIFICATIONS
- Completion of the following programs: Bachelor’s degree in Nursing and/or Psychiatric Nursing
- C.R.N.A registration in good standing
- 3 years of recent related experience or an equivalent combination of education, training, and experience (preferred).
- Current CPR and First Aid is required.
- Mental Health experience is a strong asset.
- Clear criminal record security clearance with Vulnerable Sector included (no more than 6 months old)
- Current practicing registration with the applicable provincial registering body.

Responsibilities:
- Recruits, leads and supervises the interdisciplinary team to ensure effective, consistent, and comprehensive resident care planning and delivery based on residents’ choice and PPSL policies and procedures
- Ensures holistic, comprehensive assessments of the residents and families are completed on admission and ongoing basis for individualized care planning and delivery.
- Provides leadership through a respectful, collaborative approach to monitor, supervise and direct the interdisciplinary team in care provision. Assists with new staff orientation and routine performance appraisals as allocated. Offers staff mentoring, education and/or discipline as required up to and including termination
- Provides ongoing assessment, support, treatment, crisis intervention, and case management to those Residents who have complex care needs in conjunction with the Resident LPN Managers and LPNs, who are unstable or have rapidly changing mental health status or situations
- Acts as a role model by demonstrating professionalism, empathy, trust and respect in all interpersonal relationships, and recognizes personal impact on team dynamics
- Collaborates with members of the PPSL interdisciplinary team or outside agency as needed to ensure effective, consistent, and comprehensive care planning actions and delivery based on Resident centered care and choice and PPSL policies and procedures
- Supports staff in advocating for resident choice and autonomy in decision-making and care planning. Offers ongoing education and advocacy to the residents and families, staff and volunteers
- Ensures applicable staffing levels. Monitors interdisciplinary team collaboration and processes to ensure quality resident-centered care and addresses issues as required
- Takes the lead in the PPSL Home’s quality improvement/performance measurement initiatives and processes including follow-up with summary and action plan implementation to meet or exceed best practice and Accreditation Standards
- The Care Manager will be required to carry the on-call phone on a rotational basis to assist the staff with emergent/urgent situations
- Ensures all documentation of assessments, records of observation, care provision and resident responses are according to professional standards and PPSL policies and procedures

- Presents and/or attends in-service and other educational programs as required to maintain current clinical competence and knowledge
- Performs other related duties as assigned
- Is responsible for the development, implementation and ongoing operation of the community.
- Works in collaboration with the interdisciplinary team to promote health and safety awareness, safe work practices and monitors nursing staff for safe, competent professional practice.
- Assist with various care programs as needed eg. Wound care, flu clinics, monthly audits, new projects as allocated by the Director of Care.

- Identifies and acts on the need to change and improve the clinical environment. Develops and monitors implementation plans for change in collaboration with the Resident LPN Manager. Explains the need for change and growth in terms of community goals – assists staff in understanding and coping with change and focuses on the implementation of changes in a manner that maintains and improves community service levels
- Facilitates staff participation and engagement in problem solving and develops a positive work environment.
- Sets high standards for personal and professional conduct of the nursing department.
- Liaison with physicians and other members of the healthcare team as needed
- Provides clinical support, knowledge and assistance to care staff
- Participates in assessments, by reviewing the resident’s chart and collaborating with the interdisciplinary team to ensure person-centered care using the Resident Day.
- Collaborates and provide ongoing support to Resident LPN Managers with complex resident and care staff issue.
