POST SUMMARY:
The purpose of the Clinical Case Management Specialist position, working in partnership with College Health and Disability Services, is to provide holistic and comprehensive case management services to students with mental health issues. This role is designed to enhance students’ academic and personal success by addressing their unique needs effectively. Key purposes of this role include:
1. Comprehensive Case Management: Managing the multifaceted needs of students with emotional, psychological, social, academic or personal development needs and conducting assessments, creating individualized support plans to support their academic journey.
2. Collaboration with Case Managers: Collaborating with Case Managers in Disability and Health Services to identify and support students with mental health needs, ensuring they receive seamless care addressing both mental health and academic requirements.
3. Clinical Assessment: Conduct thorough assessments of students with emotional, psychological, social, academic or personal development needs to understand their unique needs, challenges, and goals.
4. Crisis Management: Assisting with crisis management for students with complex mental health difficulties, including participation in the Fitness to Study meetings.
