The post holder will deliver quality evidence-based care to clients within their clinical area of work e.g. clients home, residential homes and other clinical environments under agreed Trust policies/guidelines.
The post holder will work under the direction and supervision of the trained Community Nurse and report directly to the Team Leader (District Nurse).

  • To undertake patient care delegated by the community staff nurses, where deemed competent to do so by the team leader in line with trust policies.
  • Provide clinical care and advise identified patients/clients.
  • To be responsible for reporting to the community staff nurses any changes or concerns in the patient/client condition or social circumstances.
  • To work independently under supervision in line with trust policies.
  • To work flexibly and relieve colleagues/rotate within the Trust as required and requested by the line manager to meet the needs of the service.

Oxleas offers a wide range of NHS healthcare services to people in community and secure environment settings. Our services include community health care such as district nursing and speech and language therapy, care for people with learning disabilities and mental health care such as psychiatry, nursing and therapies.   • To record treatment and care carried out in the patient’s/client’s home on appropriate documentation. Records must always be kept up to date and accurate in accordance with trust policies.
  • To report any concerns regarding any aspect of the patient care to the community staff nurse/ team leader in a timely manner.
  • To keep a diary for the purpose of planning work and recording all visits, both effective and non-effective.
  • To complete and submit expense forms within one week of expected completion date, to complete statistical and any other returns by a given date in accordance with trust policies.
  • To be responsible for handing over to the Health Care Professional before going off duty, detailing care provided to patients.
  • To work towards maintaining and establishing good relationships with GP’s colleagues, patients/clients and carers.
  • To assist in the orientation of staff where appropriate.
  • To be sympathetic and empathic in their approach to patient centred care.
  • To be able to work appropriately with any barriers to communication: older people, physical disabilities and cultural backgrounds.
  • To inform line manager of workload pressures that may adversely affect the level and quality of service provided.
  • To have the ability to interpret sensitive information and communicate with a sympatric and empathic approach to client centred care.
  • Ability to use Information Technology including Microsoft Office, email and the internet.
