JOB SUMMARY:
Under the direction of the Department of Medicine and in collaboration with the interdisciplinary team of Jersey City Medical Center staff and physicians, the Palliative Care Nurse Advanced Practice Provider is charged with providing high quality palliative care services, which meet regulatory and fiscal requirements to seriously ill patients in the hospital setting. Included in this responsibility is developing and maintaining strong relationships with physicians, hospital staff, community stakeholders, and system peers to deliver palliative care to seriously ill patients. Specific duties may include; formal and informal education and support to staff, patients and their families; consultation on complex cases; appropriateness of care or level of care, and liaison to internal and external contacts. In addition, this position is responsible in working with patients and families, hospital liaisons, case management staff, and physicians in assisting with decision-making, problem solving, and determining appropriateness for palliative and/or hospice services. Ability to relate and maintain a rapport with all levels of professional personnel.
Palliative Care Advance Practice Nurse will work in a collaborative and interdisciplinary environment to provide specialty level palliative care expertise to seriously-ill and medically complex patients on the palliative care service
JOB QUALIFICATIONS:
- Current Licensure as an Advanced Practice Nurse in the State of New Jersey is required
- Or Current Licensure as an Physician Assistant in the State of New Jersey is required.
- CDS, DEA, BLS & ACLS Certifications.
- Graduate level palliative care training preferred.
- Specialty level certification obtained within two years of hire and maintained throughout employment.
- 1-3 years palliative care experience preferred or experience with complex disease management and advance care planning in the acute care/hospital setting.
- Excellent communication and interpersonal skills, both verbal and written.

Responsibilities:
- Demonstrate advanced clinical palliative care skills and apply specialized knowledge for the care of seriously ill and medically complex patients.
- Design strategies and lead development of evidence-based plans for meeting patient, family, and provider needs
- Perform palliative care consultations as requested by the medical staff/referring providers, based on established departmental protocols.
- Facilitate goals of care and advance care planning conversations.
- Support the continuity of palliative care plans by assisting with discharge planning (in the role of a consultant) and providing patient and family education/teaching
- Facilitate access to appropriate resources.
- Facilitate communication amongst healthcare team members and patients/families.
- Establish a plan of care for palliative care patients based on the physical, emotional, psychosocial, and spiritual needs of patients and their families.
- Provide patient and family education regarding disease process, treatment, prognosis, symptom management, discharge planning, and end of life care.
- Provide an extra layer of support to palliative care patients and their families
- Assess and provide recommendations for the management of pain and non-pain symptoms in the setting of serious medical illness, as they relate to palliative care
- Identify and address ethical issues in complex care situations
- Use evidenced based medicine in the care of patients
- Educate professional stakeholders (physicians, leaders, and other hospital staff).
- Participate in weekly interdisciplinary team meetings and staff meetings.
- Participate in palliative research and/or QA/QI projects.
- Perform a patient record review with the collaborating physician on a bi-annual basis.
- Perform accurate and timely billing of APN services.
- Serve as a role model for palliative care delivery.
- Maintain clinical competence in Palliative Care
- Maintain a professional demeanor and actively seek opportunities to increase knowledge and skill level
- Function independently as a nurse practitioner, with a physician in a collaborative practice relationship.
- Accept constructive feedback and demonstrates willingness to learn and improve
- Establish collaborative relationships within and across departments to promote excellent patient care.
- Demonstrate flexible and productive work habits and effective time management skills.
- Participate in quality improvement initiatives and developing evidence-based guidelines.
