Description:

Are you currently a band 5 looking for development opportunities or a band 6 with a specialist interest in integration, hospital discharge and flow. This is an exciting time to join the integrated discharge team, we have a range of transformational projects we are developing that you could be part of.
We welcome applications from those Returning to Practice, as well as those seeking full and part time working hours.

To assess, plan and coordinate safe, timely discharges via the most appropriate discharge pathway to meet the patients’ required needs.
Manage a caseload of patients with varying diagnosis and or /complex needs with a range of disabilities including physical, learning and mental health disabilities.
To educate and train the team on the role of OT and provide day to day management for junior staff, ensuring supervision and annual review.
To participate in the planning, development and evaluation of the discharge team service within the acute trust and the wider integrated services.
Take a lead for information management to ensure high quality data which is recorded, evaluated & reported in a timely manner.
