Description:
JOB OVERVIEW: Reporting to the Health Director, the Reaching Home Program Coordinator (RHPC) wil provide aid to unhoused community members. The incumbent is expected to build relationships with unhoused program participants to provide emotional support, supplies, and support access to services. The RHPC will also work collaboratively with other Driftpile programs and agencies to address the community member needs.
QUALIFICATIONS
- 2-years of experience in outreach or a related position.
- Certificate, diploma or degree, two years’ experience working with vulnerable populations, or a combination of equivalent experience and/or training.
- Demonstrated commitment to social justice, equity, and advocacy for vulnerable populations.
- Ability to work both independently and collaboratively within a team environment.
- Willingness to work flexible hours as needed.
Responsibilities:
- Assist participants in their quest for establishing a better quality of life through support and encouragement, as well as assisting with life skills and employment goals.
- Provide information to participants regarding programming and other community services.
- Refer participants to appropriate resources, such as treatment, mental health services, health services, food banks, shelters, employment services, etc.
- Provide necessities as needed (food, clothing, blankets, hygiene kits, etc.).
- Transport program participants to food banks or other services as needed.
- Collaborate with community organizations, government agencies, and other stakeholders to coordinate wrap-around supports for individuals.
- Assist with volunteer placements, pre-employment, and employment services.
- Administer wellness cheques and support dollars to help maintain housing.
- Provide infromation about housing and act as a mediator between the community members and property owners. Facilitate communication as necessary.
- Maintain and manage all documentation of interactions with community members and progress of housing status’.
- Complete and provide timely reports to the Director of Health & Healing.
- Gather and compile statistics pertaining to the unhoused community members.
- Participate in community and team meetings, extensive networking with community agencies to collaborate and advocate for clients.
- Ensure compliance with policies, procedures, and regulations.
- Other duties as assigned.
