POSITION PURPOSE To work within a flexible, interdisciplinary structure, that ensures the delivery of high quality rehabilitation services to all clients of HIP services, making sure that the client’s needs are met and they receive ‘the right treatment in the right place at the right time’. To provide clinical and service leadership in relation to Social Work within Community Rehabilitation, including leading service improvement changes. Clinicians are required to provide assessment and management of clients in a number of settings across the continuum of care within HIP, including client’s residences, community settings, the HIP Centres or Specialist Clinics POSITION DUTIES Assess and manage all assigned clients, including completion of comprehensive psychosocial assessments, according to HIP and St. Vincent’s policies and procedures and relevant rehabilitation protocols Deliver client focussed, high quality social work assessment, intervention, education and management Actively identify and advocate for social work intervention in clients of community rehabilitation and educate other clinicians on the role and scope of social work within community rehabilitation Provide collaborative care coordination for clients and carers as required, in the context of their health, functional, psychosocial, cultural, environmental and economic needs.
