3. QUALIFICATIONS
An associate degree, Advanced Diploma, Diploma, or equivalent qualification in a related field
· Relevant experience of at least three (3) years (preferred but not essential)
· A current NDIS Worker Screening Check
· A current Working with Children Check
· A current First Aid Certificate

4. COMPETENCIES AND EXPERIENCE
Ø You must have relevant full-tme work experience or bachelors degree.
Ø Previous experience supporting People with Disability.
Ø Demonstrated proficient written, verbal and interpersonal communication skills.
Ø Demonstrated ability to work with minimal supervision, and collaboratively as a member of a multidisciplinary team.
Ø Ability to effectively priorities and organize own work / caseload.
Ø Demonstrated commitment to professional development, quality improvement and evidence-based practice.

· Strong interpersonal and communication skills
· Excellent organizational and observational abilities
· The ability to work independently with professionalism and empathy

· NDIS experience (highly regarded but not essential)
· Qualifications in Disability, Aged Care, or Mental Health (highly regarded)
· A background in Early Childhood or Education is a bonus
2. MAIN DUTIES/RESPONSIBILITIES
Ø Support clients to participate in age-appropriate activities and tasks including employment, in home support and community access.
Ø Support clients to access activities and outings in the community.
Ø Provide direct assistance to each client in accordance with their service plan and as directed
Ø Work in accordance with the medication administration policy, work health and safety procedures and client right’s policies including privacy and confidentiality.
Ø Monitor the safety and well-being of the clients and report any concerns to the Manager.
Ø Ensure completion of appropriate documentation. Relevant information on support provided is to be recorded in client notes and communication book as required.
Ø Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
Ø Work with clients, family/ carers, relevant case managers and other professionals to support the client’s individual goals.
Ø In conjunction with other members of staff assist in developing, implementing and evaluating training and behaviour programs to attain identified individual goals.
Ø Ensure that all programming undertaken is appropriately maintained, documented and reviewed.
Ø Assume the role of key worker for designated clients, including the preparation of a monthly report on client progress.
Ø Liaise extensively with other staff to ensure the overall smooth and consistent running of the service.
Ø Report regularly to manager
Ø Actively contribute to a team environment through open communication, participation in regular staff meetings, planning processes and policy review.
Ø Provide back up and support to other staff, as necessary.
Ø Participate in Performance Development meetings to identify ongoing training and support opportunities.
Ø Notify the service if you are unable to work your rostered shifts, with as much notice as possible.
Ø Contribute to Case reviews with Employment Consultants and Facilitators.
Ø Take reasonable care for his or her own health and safety
Ø Take reasonable care that his/her acts or omissions do not adversely affect the health and safety of other persons.
Ø Report any quality and safety incident or hazards at work to their Manager or Supervisor.
Ø Carry out their roles and responsibilities as detailed in the relevant policies and processes.
Ø Keep them self fully informed of the contents of the NDIS system and apply it to daily work
