Description
A Systems Navigation Coordinator (SN Coordinator) is a person with experience in systems navigation, an understanding of legal and ethical issues, and counts with a bachelor’s in social work, psychology, or related field. This individual is responsible for assisting new and existing patient navigators on Health Risk Assessment (HRA) administration and documentation, resource and referral documentation, patient-centered communication, and the organization’s care team structure. In coordination with the Associate Director of Health Equity, this position will develop and disseminate workflows to support internal (e.g., BH, Pharmacy, RD, Health Ed., etc.) and external (e.g., food banks, government assistance programs, etc.) systems navigation and care coordination. The SN Coordinator will monitor and report on various quality metrics (e.g., PHQ-9, SBIRT, PRAPARE, SMG) to support continuous quality improvement. This individual will train and support Patient Navigators working on various projects and deliverables throughout the organization. REPORTS TO: Associate Director of Health Equity QUALIFICATIONS:  1. Minimally Bachelor’s degree in Social Work, Psychology or related field. 2. Minimum one year experience in health education program and/or systems navigation.  3. A working knowledge of legal and ethical issues affecting systems navigation and health outcomes. 4. Thorough working knowledge of business English, spelling, punctuation, and contemporary general office practices and procedures 5. Math skills sufficient to prepare routine spreadsheets, calculate/balance invoices/expense reports, and similar  6. Basic leadership skills.. 7. Computer skills in Microsoft Office programs (Word, Excel, etc.), electronic health record systems, and database systems. 8. Typing/data entry skills sufficient to meet production requirements. 9. Effective verbal and written communication skills to communicate clearly and effectively with patients and others.  10. Very effective oral and written communications skills including public speaking experience. 11. Effective work organization skills.  12. Ability to work effectively as a team player. 13. Ability to be flexible and work in a changing environment. 14. Sensitivity to the different cultures represented among members and staff. 15. Demonstrated ability to listen and communicate with others in a professional and caring manner including sensitivity with individuals from diverse cultures and lifestyles. 16. Demonstrated ability to set priorities for tasks to work effectively in spite of interruptions and under minimal supervision.    Self-starter, reliable and dependable.     17. Demonstrated proficiency with the electronic health record database within three months of attending training session(s).    18. Fluency in English (speak, read and write). 19. Ability to solve problems and make routine recommendations. 20. Ability to maintain absolute confidentiality about health care and other patient/client information. 21.
