Facilitate referrals and assist in discharge planning when social work intervention is no longer needed. Caseload and Record Management: Efficiently manage caseloads and maintain treatment records in compliance with the Ontario College of Certified Social Workers and Social Service Workers standards. Client Support and Education: Use a client-centered approach to address psychosocial issues, provide relevant educational resources, and participate in quality improvement initiatives. Professional Development and Collaboration: Engage in ongoing professional development, maintain confidentiality, and build positive relationships within the interdisciplinary healthcare team. Education and Certifications Required: Social Service Work diploma Current Member (in good standing) of the Ontario College of Social Workers and Social Service Workers.
