As a Mobile Case Manager, you will play a vital role in guiding individuals through their recovery and return-to-work journey following a workplace injury. Your focus will be on positively influencing and encouraging all parties to actively participate in the return-to-work process and take ownership of their actions. How you'll make an impact What You’ll Do: Facilitate face-to-face visits with injured workers, employers, and health professionals to support recovery and return-to-work plans. Educate and guide stakeholders on the return-to-work process, legislative requirements, and available support services. Attend medical reviews and collaborate with vocational providers to implement strategies for early intervention and successful outcomes. Develop and monitor Work Plans (Service Plans) to achieve sustainable return-to-work goals. Maintain accurate documentation to support evidence-based decisions and ensure compliance with claims management processes.
