ABOUT KIMBERLEY RENAL SERVICES

Kimberley Renal Services (KRS) includes 4 Renal Health Centres based in Fitzroy Crossing, Broome, Kununurra, and Derby and a mobile prevention unit.
The incidence of Kidney Disease in the Kimberley is one of the highest in Australia. Chronic Kidney disease (CKD) and End-Stage Kidney Disease (ESKD) incidence within the Aboriginal population of the Kimberley greatly exceeds the national burden of disease. Dialysis prevalence for this region has more than tripled in the last decade and is increasing at a much faster rate than in the rest of Western Australia (WA).

ABOUT THE OPPORTUNITY

Kimberley Renal Services now has an opportunity for an Aboriginal Care Coordinator to join their KRS team in Kununurra WA, on a Full Time basis.
Reporting to the Integrated Service Manager you will assist patients with activities of daily living whilst attending the Renal Health Centre.

Some of your key responsibilities will include:

  • Establishing and implementing effective care coordination services within the region to support Aboriginal patients to seek more proactive chronic disease management and care.
  • Provide relevant information and education about renal illness and treatments and assist clients to access services.
  • To engage with Aboriginal renal patients in outreach home setting.
  • Increase cultural awareness for renal staff new to the area of Aboriginal health.
  • Work within the multidisciplinary renal team and advocate for the rights of renal clients and their family.
  • Develop and implement strategies to improve access to mainstream primary health care for Aboriginal and Torres Strait Islander people.
