If you are passionate about transforming lives and making a real impact on people’s health and well-being, this is an exciting opportunity to work with primary care to engage and support families living in Bootle, Seaforth, Litherland, Crosby and Maghull. As an ACEs health and well-being coach, successful applicants will champion and deliver the adult ACE’s recovery toolkit programme. Using a combination of creative activities and group work, providing psycho education and pro social modelling to develop participants resilience giving them opportunity to experience the healing of relational support.

Successful applicants will join South Sefton Primary Care Network’s Mental health hub, as part of a dynamic, developing service partnered with Sefton council. And will have the right skills and values to be an exceptional health and wellbeing coach. And to provide and continue development of a nurturing, safe and stimulating creche, working across a range of community sites
Our health and wellbeing workers will use health coaching skills to support people to develop the knowledge, skills, and confidence to become active participants in their care so that they can reach their own health and wellbeing goals. They also provide access to self-management education, peer support and social prescribing.
Work alongside people to coach and motivate them through multiple sessions, supporting them to identify their needs, set goals, and help them to implement their personalised health and care plan. Activities are delivered through appropriate models and frameworks to engage, empower and enable residents to take ownership of their own solutions and develop resilience.
To work jointly with the Primary Care Network and Local Authority Family wellbeing hubs to work with adults and their families to deliver high quality interventions, primarily in group settings, focussing on holistic and temporary support to achieve agreed outcomes. including creche provision.
Promote engagement of the universal offer through the community, voluntary and faith sector organisations.
They will also utilise appropriate marketing techniques to promote awareness of the service and represent the service at events as required. including agreed COMM’s / social media.
They will lead on the provision and development of the Creche supporting families accessing the Adult ACE’s RTK programme.

South Sefton PCN is a large Primary Care Network, covering the four localities in south Sefton with a population of around 155,000 residents
PCNs are groups of GP practices working with a wide range of partners to improve the health of their population. Key to this purpose is strengthening general practice provision through working together, where it makes sense to do so and working as part of integrated care teams which work collaboratively for and with local people in each locality.
The PCN is participating in an innovative programme with Sefton Council to integrate our offer of proactive support to residents.
Job Responsibilities
Work with GPs and the PCN team to identify and engage a case load of clients that would benefit from very early help to proactively support family or household wellbeing:
Manage and progress clients to achieve positive outcomes based on a solution focussed approach.
Assess and identify levels of risk, vulnerability and safeguarding using prescribed assessment tools, and escalating to GPs as necessary.
Deliver targeted activity working either on a 1-1 basis or in groups using a holistic approach to address the root causes in line with the localities outcome framework and health and wellbeing coach framework.
Maintain accurate records of all interactions using the PCN or council systems as required.
Undertake home visits to engage participants and ensure that Sefton residents are safeguarded within the community.
Increase parental engagement to strengthen family relationships and support children’s education
Work co-operatively with colleagues and partners to share information, ensuring a joined up approach.
Ensure Quality Assurance Framework, legislation, regulations, policies and procedures are central to all good practice.
Engage in the development of the service and CPD through PDR, training team/service meetings, and supporting raising awareness sessions across the partnership on key and emerging issues.
Maintain quality Creche provision and work in partnership with families to support parental understanding of their role in their child’s development by modelling high quality early education practice within the creche.
Signpost parents to activities within the community to build resilience within the family. Support family members to develop skills to enable them to carry out household tasks such as budgeting, supporting health and hygiene, increasing personal and parental confidence and capacity.
Utilise appropriate marketing techniques to promote awareness of the service and represent the service at events as required. including contributing to COMMs / Social media.
Ensure that client information data is lawfully gathered, accurate, up to date and only divulged in accordance with GDPR
