ACP Elderly Virtual ward/ Front door frailty

JOB PURPOSE The post holder will provide clinical expertise and leadership to the integrated clinical team, drawing on clinical knowledge to undertake advanced assessment, diagnosis and treatment, inclusive of independent prescribing The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the Care of the Elderly Virtual Ward MDT team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. The post holder will provide support and facilitation to trainee nurse practitioners and will provide a consultancy and advisory function to the MDT, utilizing expert knowledge and skills. The role will also provide a teaching and education service to students, medical and nursing staff.
The post holder will be responsible for providing support to the development of an integrated care pathway. Liaising with social care and primary care colleagues to support admission avoidance. in liaison with other clinicians and partner organisations. The post holder will be responsible for the operational management, performance monitoring , professional leadership, service improvement and development of the virtual war
