The PACE Activities Coordinator II is responsible for the evaluation, planning, implementation and documentation of the overall adult day health activities of the PACE Adult Day Health Center. Under the direction of the PACE Center Manager, The PACE ACII develops and implements individualized care plans of PACE program participants. The purpose of enrichment activities is to promote socialization and improve participants’ physical and cognitive skills. The AC II develops enrichment activities such as insight-oriented groups, leisure education, out-trips to the community, crafts, and workshops, and a variety of recreational activities. The ACII develops activities based on cognitive, physical and emotional or spiritual modalities. The PACE AC is a CMS-required member of the PACE Interdisciplinary Team (IDT) and is responsible to participate in team decision-making to authorize and approve medically necessary products and services based on the written plans of care. The PACE program’s vision is to provide individualized and joyful care through exemplary teamwork serving as many seniors as possible with the best quality-of-life in their communities. PACE offers seniors and their families the care, nutrition, rehabilitation, transportation, and supportive services they need to remain healthy so that they can live in their own home
QUALIFICATIONS
- High School Diploma - Required
- Associate’s or Bachelor’s Degree- Health Care Related Field- Preferred
- Activity Director Certified (ADC)-Required
- 1 Year Working with Elderly and Frail Population - Required
- 3 Years Health Care Experience in Social or Recreational Programming- Preferred
- Driver’s License - Required
