Description:
Role Summary
- Form a key member of the senior management team of the Acute Care at Home Service, flexing roles to address new challenges as they emerge, and engaging in developing and transforming the service
- Working collaboratively with other members of the triumvirate will lead, the Clinical, Operational and Financial Performance of the Acute Care at Home Service i.e., Community Rapid Intervention Service (CRIS), Virtual Wards (VW) and the Acute Visiting service with regard to attendance and admission avoidance; ensuring resources are used to maximum efficiency whilst delivering high quality, safe and efficient care for patients across Staffordshire and Stoke on Trent Integrated care system.
- Provide proactive, compassionate, system collaborative and visible Clinical leadership Ensuring that there are appropriate systems and processes in place to deliver the strategic objectives and drive forward the service to improve the Clinical effectiveness of the Service.
- Provide advice and support to Directors demonstrating a commitment to team working and shared responsibility for the achievement of the aims and objectives of the collaborative.
Key Responsibilities
- To provide senior professional clinical leadership to the service.
- Be part of a Consultant of the week rota, providing oversight and governance to CRIS and VW
- Within the triumvirate, provide leadership to Acute Care at Home and contribute to developing the strategic direction of the service.
- Provide strategic advice and leadership and contribute on medical and clinical matters to the Acute Care at Home Board and sub committes.
- Accountable for the clinical governance within the service in partnership with the Head of Nursing.
- Work collaboratively with the clinical leadership within the service to ensure the service delivers against all pillars of Clinical governance and manage the Clinical Leads.
- Take shared corporate responsibility for the financial performance of Acute Care at Home, including the achievement of financial targets, balancing the potentially conflicting demands of budgetary requirements and clinical standards

- Undertake proactive horizon scanning for either developments relating to Acute Care at Home work or opportunities for Acute Care at Home involvement around health issues
- Support the delivery of systems,processes and personal values that will support the Trust’s sustainable development objectives with regard to issues such as Carbon reduction and waste minimisation; and to encourage all stakeholders of the Trust to act as enthusiastic agents of change.
- Represent Acute Care at Home at various forums, developing partnerships, sharing best practice and integrating this knowledge.
- Ensure there is clinical engagement in the day to day activities of Acute Care at Home, and in performance improvements and service developments, working with the leads in other services across the Trust.
- Promote innovation and quality improvement to ensure best-practice services for patients and positive working experience for staff.
- Support the Acute Care at Home annual budget setting process and delivery of quality and performance metrics.
- Promote a just and learning culture based on openness, learning, coaching and continuous improvement and support continuing personal development
- Promote an environment that improves health, well-being, safety and security for staff
- Maintain personal and professional development to meet the changing demands of the job, participate in appropriate training activities and encourage and support staff development and training
- Take action when professional standards fall short of those acceptable
- To carry out employee relations matters, such as disciplinary or grievance, in line with Trusts policies as required
- Through the tirumerverate partnership with the Head of Nursing and Operations ensure strategic and annual plans for Acute Care at Home are developed and contribute to the development of the systems strategic and annual plan.
