Description:

An exciting opportunity has arisen for a Band 5/6 development post for a clinician in the Older Adult Dementia Home Treatment Team.
DHTT is an acute assessment and home treatment team for older adults experiencing problems around their diagnosis of Dementia, although at times we have patients that have dual diagnosis.
These posts can be challenging, fast paced and offer excellent opportunities for professional development in enhancing skills, and the excitement of looking at working in a more dynamic way with supporting people in their own homes and where possible avoiding hospital admission, while maintaining and not deskilling those under our care. Where recovery is limited such as in organic Mental Health, the clinician will be supporting the person, care providers, and or the carers in managing behaviours that challenge and medication reviews as and when required. Offering intervention-based support to reduce behaviours that challenge and supporting the person and their loved ones/carers to remain in their current place of residence, whether that be within a care environment or their own home. DHTT also work collaboratively with other services, and are renown for providing information and referrals to other services to support patients and their families.
The post holder will be responsible for the urgent assessment including risk assessment, planning, and implementation of evidence based care to service users with dementia who have complex health/behavioural needs and their carers within the service catchment area. The post holder will provide professional advice and support to other agencies and members of the multi-disciplinary team.
The primary purpose of the service is to provide a timely intensive home assessment and treatment service people with a diagnosed or suspected dementia who are presenting with behavioural and emotional disturbance to a degree where the risk they pose indicates that an inpatient admission may otherwise be required.
The post holder will assess care needs and develop, implement and evaluate programmes of care for those who are experiencing acute behavioural and emotional disturbances in dementia within agreed limitations.
When required the function of Admission Discharge coordination will be carried out.
Patterns of working across 0800-2200hrs Monday to Friday and 0900-1700 at weekends and bank holidays.

To provide a high standard of clinical care ensuring safe and effective assessment, treatment and comprehensive discharge planning using highly developed clinical reasoning skills and appropriate assessment tools.
To work flexibly across a 7-day-a-week service.
To delegate aspects of the patient treatment plan to other members of the team, whilst maintaining overall responsibility for patients care, where appropriate.
To ensure the clinical caseload and clinical practice of team members is of the highest standard of clinical care.
To manage case-load and service delivery in accordance with and in order to contribute towards the attainment of Service/Trust productivity requirements.
Ensure up to date agreed care plans, risk assessments and reviews are in place.
Work with individuals and their carers to identify their related needs and circumstances; and enable them to understand and manage within their own homes.
Plan, implement, review and improve interventions to meet people’s identified needs and manage their inherent risk.
Following assessment to “work outwards” from the immediate crisis and help the service user stabilise and help the carers and extended system feel more confident about how they manage the immediate situation and any continuing difficulties.
Protect people from abuse, neglect and harm.
To work collaboratively and promote effective working relationships with members of the multi-disciplinary team, ensuring effective and appropriate clinical decision-making, both within the Older People and Frailty Division and with external agencies.
Provide management supervision to junior staff in line with the Trust policies and protocols for supervision, appraisal and mandatory training.
To work collaboratively as a part of the clinical leadership team to provide consistent leadership across the service.
Day to day management of staff and coordination of the clinical activity and workload.
Supervise the work of junior staff, providing assurance of the quality of interventions, record keeping, care planning and risk assessment.
Day to day supervision of staff to ensure that all are working within the service model and to achieve the targets and performance of the service within the wider Trust priorities and strategic direction.
Contribute to, and where requested, lead on the development of local service protocols
