Description:
Establish and lead the handover reports for the HOSPITAL AT NIGHT team. Initiate, lead and coordinate management of patients identified as clinically at risk, referring to appropriate colleagues when necessary. Ensure timely and appropriate care is delivered by the relevant health care professionals. To undertake a comprehensive assessment of patients, participate in the assessing, planning, implementation and evaluation of individualised programmes of care.
Co-ordinate clinical emergencies i.e. cardiac arrest. Act as an innovative and enthusiastic role model, providing leadership, guidance and advice to staff on clinical, operational and professional issues. Demonstrates clinical expertise by role modelling patient centred practice.
Ensures that staff adhere to the systems and processes in place to support the provision of patient centred care to agreed standards. Ensures adherence to the CHFT and NMC guidelines for the safe handling, administration, storage and custody of medicinal products Ensures good communication between staff and relatives is maintained to assist in their understanding of care, treatment and progress of the patient is and implemented and adhered to. Maintains own personal and professional development in accordance with NMC Code of Professional Conduct and within the Scope of Professional Practice. Participates effectively in the multi-professional team providing an expert nursing perspective.
Contribute to the audit process in line with the National Outreach Forum Standards and utilise results in practice. Ensures staff are aware of and work in line with Trust policies. Support patient care using technology relating to task management, observations and handover
