Description:

To support the maintenance of various patient systems on recording and tracking of up to date information relating to patient pathways and including referrals. Monitor and follow up outstanding information from families and schools. Request all associated supporting documents Responsibility for chasing supporting documents. Maintenance of departmental databases and spreadsheets Use of patient administration systems to check and update patient information Ongoing liaison with families and schools regarding referral process and obtaining questionnaires.
Establishing good levels of communication General office and admin duties. Ensure that policies, protocols and procedures for the work of the department are followed Ensure that the Trusts Code of Practice for Safeguarding Patient Information is adhered to at all times.
