Description:
DESCRIPTION
An Intake Assessment Nurse RN is a registered nurse whose primary responsibility is to accept referrals from a variety of resources via phone and fax with additional responsibility of co-signing referrals entered by LVN’s and or non-clinical intake staff, as well as coordinating the initial hospice assessments with the clinical team.

REQUIRED QUALIFICATIONS:
- Completes necessary education to attain RN Licensure in state of California
- California Registered Nurse License upon hire
- 1 year of Registered Nurse experience
PREFERRED QUALIFICATIONS:
- 1 year of Hospice experience
