SUMMARY:

The Adopt-A-Senior Client Coordinator is responsible for older adult outreach and recruitment, managing incoming referrals, intake and needs assessment, assignment for case management, offer case management as needed, case record reviews and assist with planning and participating in Senior Socials and/or other projects and events. Other duties include representation of Senior Services Department and Catholic Charities in the community.

MINIMUM QUALIFICATIONS:

  • Education
  • Associate’s and/or bachelor’s degree preferably in Social Work, related social service field, or business-related field preferred but not required
  • High School Diploma/GED with relevant experience
  • Experience
  • In addition to a degree, a minimum of one-year experience working with older adults or vulnerable population
  • Experience may be substituted for degree in which case person must have at least 2 years of experience working with older adults or vulnerable population.
  • License and Credentials
  • Must be at least 21 years of age
  • Reliable transportation
  • Valid driver license
  • Valid vehicle liability insurance
  • Must have a clean driving record

MINIMUM KNOWLEDGE AND SKILLS:

  • Strong understanding of issues related to aging and social isolation of older adults.
  • Experience in delivering case management services to individuals.
  • Knowledgeable of community social services.
  • Must demonstrate organizational skills and attention to detail.
  • Ability to work without close supervision and to multitask.
  • Must have good written and verbal skills.
  • Must have good critical thinking and problem-solving skills.
  • Experience and proficiency with computer software, to include Microsoft Applications: Outlook, Word, Publisher, and Excel.

TRAVEL REQUIREMENTS:

Travel requirements for the position includes 80% local and 10% overnight.

Responsibilities:

  • Recruitment, hiring and training of all staff under their supervision and will make recommendations for termination of employees.
  • Develop and implement employee development plans for program employees.
  • Recruit older adults through community presentations, participation in older adult/volunteer-oriented events, and collaborations with community stakeholders.
  • Oversee case management services and assist AAS Case Managers with creation of older adult individual service plans.
  • Ensure that volunteers and older adult beneficiaries are appropriately matched in compliance with Catholic Charities’ policies and procedures.
  • Assist with completion of case record reviews, scoring, and summaries.
  • Maintain program statistics/record keeping through use of agency-required database(s), client files and through data entry on program-mandated tracking excel sheets.
  • Monitoring and reporting performance measures and deliverables.
  • Complete all agency and funder mandated reports on client outputs and outcomes.
  • Complete client intake and assessments as needed.
  • Other duties as assigned by administration, i.e. Senior Director, VP of Programs, and/or COO/CEO.
