Description:
QUALIFICATIONS
- Education & Experience:
- Post-secondary education at community college level in the social sciences and behavioral disciplines.
- Experience (preferably 3 years) working with individuals with developmental disabilities.
- Knowledge and/or Skill Level:
- Knowledge of normal and abnormal development
- Knowledge of family systems theory and an understanding of the family development and functioning
- Ability to develop trusting, co-operative relationships with clients, families, service providers, and other stakeholders
- Excellent communication, negotiating skills and judgment to manage communication
- Knowledge of community resources for youth and adults with special needs including dual diagnosis and relevant legislation.
- Ability to support self-advocacy and empowerment for individuals/families.
- Strong knowledge of strength-based planning and interventions.
- Sensitivity, understanding and ability to work with a variety of value systems, beliefs and cultures.
- Working knowledge of word processing and data entry.
- Capacity to work independently.
- Aware of Health & Safety Rules & Regulations.
- A working understanding of the French Language is beneficial.
Responsibilities:
PURPOSE OF POSITION
To provide service using wraparound principles, to adults with development disabilities living within their particular community setting, to assist them to lead lives which reflect their values and choices.
STATEMENT OF MAJOR RESPONSIBILITIES
Adults and their Families:
- To establish and maintain a positive rapport and professional relationship with each client
- To assist individuals (and their families) to identify their priorities using a strength-based approach
- To promote the rights of individuals to participate meaningfully in their community
- To provide service co-ordination/case management by:
- assisting to access community services and resources
- identifying the need for specialized services and assisting with referrals
- assisting to develop individual support plans, transition plans
- coordinating services provided by multiple agencies
- assisting families to recruit mediators and respite providers
- assisting families, community and agency personnel to identify strategies to respond to gaps in service
- To provide consultation and information to families and other service providers
- To provide direct service (if required and in the absence of other readily available services) such as: court support and assistance with the justice system, problem solving, empowering, budgeting and assistance to manage personal finances, assistance to find suitable accommodation, assistance to attend appointments
- To facilitate groups (such as peer support, relationships, social skills)
- To link families with similar concerns and needs
Community:
- To maintain a co-operative working relationship with professional agencies and community resources
- To strengthen communities to respond to the needs of vulnerable populations
- To participate in inter-agency committees as assigned
- To promote the Community Resource Service in the community, through presentations, literature and inter-agency functions
Agency:
- To regularly attend and participate in supervision meetings and annual performance appraisal
- To regularly attend and participate in team meetings, including construction of agendas, chairing meetings and recording minutes
- To adhere to all agency standards, policies and procedures
- To collect and report service delivery data as required
- To record and report client information necessary for effective case management
- To participate in staff training in consultation with manager, and to demonstrate initiative in learning and developing new techniques of service delivery
- Must be aware of and adhere to Health & Safety Policy Rules & Regulations
- To carry out such other duties as may be assigned
