GENERAL REQUIREMENTS
- Provide casework and follow-up services to substantiated victims and vulnerable adults to ensure long-term support and intervention to prevent further abuse, neglect, and/or exploitation of the victim.
- Other duties as assigned within the guidelines of this position.
Responsibilities:
- Receive and document reports of abuse, neglect, or exploitation of older adults and persons with disabilities.
- Conduct a case conference with the identified protective services supervisor when developing the care plan, at case closure and at any other time deemed necessary by supervisor.
- Provide follow-up services as appropriate to client needs at a minimum of one face-to-face contacts and two phone contacts every three months for a period of up to twelve months following intervention.
- Document all activities on required forms adhering to time frames and policies of the APS program.
- Make referrals for appropriate follow-up services, such as case management, ombudsman or other community based services.
- Ability to relate well to older individuals and persons with disabilities respecting their privacy, dignity and right to self-determination.
- Must display strong and independent problem-solving skills.
- Ability to prioritize and respond to client needs in accordance to specified time frames.
- Ability to work effectively with co-workers and other community agencies.
- Attend staff meetings, in-services, conferences and workshops as approved or recommended by supervisor.
