Description:
The primary role of the Advanced Clinical Practitioner for Clozapine & SMI Health Checks will have operational and clinical leadership for the delivery of the clozapine pathway and delivery of SMI health checks. The post holder will demonstrate a high level of expertise within the specialist service providing advice, education and support to staff, patients, their families and carers.
Responsibilities:
THE ROLE HAS THE FOLLOWING PRIORITIES:
To practise autonomously as an advanced clinical practitioner and non-medical prescriber within the clozapine and physical health (SMI) speciality area to provide patient-centred clinical care. This will encompass the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice throughout the directorate.
Supporting new ways of working that emphasises a more efficient and patient focused service, and will ensure the safe treatment, referral and discharge of patients.
Responsibility for leading the on-going development of clinical practice and standards of care within the clozapine service in collaboration with the lead pharmacist, including the development of policies, procedures, protocols and guidelines in collaboration with multidisciplinary colleagues.
Delivering a 20% clinical component and 20% related to appraisal, clinical audit, teaching, continuous professional development and research, and 60% operational leadership for the Clozapine service
THE MAIN DUTIES OF THIS POST ARE TO:
Contribute to the delivery of specialist interventions, focus ing on clozapine & physical health/wellbeing
Deliver medicines optimisation in collaboration with the lead pharmacist for clozapine, using a person-values centred approach.
E nsuring 90+% achievement of P hysical Health Checks for patients with an SMI
Demonstrate expert clinical knowledge through provision of highly specialist advice and support to patients, families, BHFT staff and external stakeholders
Contribute to development and implementation of the service strategy, performance metrics, key objectives and efficient use of resources.
Undertake and support medicine and physical health-related audits and quality improvement projects
Lead on implementation and support the use of shared care protocols where appropriat e

THE “MUST HAVES” FOR YOU TO BE CONSIDERED FOR THIS ROLE:
Registered nurse and non-medical prescriber with a minimum 4 years’ experience at Band 7 relevant to the role
Excellent working knowledge of mental health services and collaborative working in multidisciplinary teams, with the ability to demonstrate and deliver physical health skills along with training
A passion for physical health care that matches our desire to provide excellent healthcare for people with serious mental Illness
You will be comfortable working in a busy, collaborative environment and proficient at engaging with people who experience health inequalities or difficulty accessing services.
