Are you ready to take your clinical leadership skills to the next level? We’re seeking an experienced and dynamic Advanced Clinical Practitioner to join our team and play a pivotal role in delivering exceptional patient care.
As an Advanced Clinical Practitioner with us, you’ll have the opportunity to provide dynamic and professional clinical leadership, engaging in advanced clinical activities while upholding the highest standards of care. Working closely with our lead consultant, lead nurse, Deputy Sisters/Charge Nurses, Practice Development Team, and Multidisciplinary Team, you’ll collaborate to ensure each patient receives the tailored care they deserve, enhancing their overall experience.
Uphold Excellence: Ensure top-tier patient care standards, catering to each person’s needs for optimal health outcomes.
Direct Patient Engagement: Perform in-depth patient evaluations through face-to-face reviews, whether in person or virtually, to gauge needs and potential progression towards further care.
Virtual Patient Interactions: Connect with patients via phone or video, providing necessary support and guidance.
Attend Daily Virtual Board Meetings: Participate in virtual rounds to stay informed and contribute to patient management discussions.
Virtual Ward Prescriptions: Issue prescriptions for virtual ward patients when necessary, ensuring timely and appropriate treatment.
Provide Guidance on Patient Care: Advise the virtual ward nursing team on patients experiencing changes in clinical status, facilitating informed decision-making.
Facilitate Smooth Transitions: Assist in patient discharge processes, collaborating with primary care and community services, and preparing discharge documentation.
Monitor Test Results: Regularly review patient test results to track progress and adjust treatment plans accordingly.
Embrace Flexible Work Dynamics: Adapt to varied work settings, including acute trust facilities, technological platforms, and patient residences, to deliver care effectively.
