Description:

This is the perfect opportunity to be part of a diverse team providing assessment, diagnostics, diagnosis, treatment, advice and referrals As an Advanced Care Practitioner, you will be considered a key person in day-to-day clinical leadership and expertise within the department. The Advanced Care Practitioner will demonstrate highly developed specialist knowledge and skills in the management and care of the Urgent/Emergency patient. You will work autonomously within your sphere of competency as a clinician in a diverse multi skilled team which includes General Practitioners, Emergency Nurse Practitioners and administrative staff. They will ensure patients are fully informed regarding their proposed treatment, provide appropriate information and support liaising with appropriate colleagues to ensure a seamless service for the patients.
The Advanced Care Practitioner will formulate clinical decisions and treatment plans to manage the urgent and/or emergency patient illness and injury. The Advanced Care Practitioner will demonstrate specialist knowledge and skills in the management of care of the urgent and/or emergency patient They will carry out holistic needs assessments of patients, initiating interventions and referring to members of the multidisciplinary teams as appropriate. They will exercise advanced clinical expertise, levels of judgement for the development and review of care protocols and training for staff within their area of specialism. Audit and monitoring of standards of patient care, using the results to influence practice, thereby continually improving evidence-based practice and delivery6 of patient focused care.
They will be required to develop and maintain links with local and national strategic organisations
