Description:

Scope and Purpose Deliver a high standard of patient case as an Advanced Care Practitioner in General Practice, using advanced autonomous clinical skills and a broad and in-depth theoretical knowledge base. Manage a clinical caseload dealing with presenting patients’ needs in a primary care setting. Develop the clinical services offered by the practice. Job Dimensions Mentor and support other clinical staff in developing and maintaining clinical skills.
Manage own clinical workload in general practice, responding effectively to patient need and ensuring patient choice and ease of access to services. Lead the implementation of clinical contractual needs of practice, including Quality and Outcome Framework and Enhanced Services. Primary Duties and Areas of Responsibility Clinical Role Provide the choice of direct access to Clinical Practitioner both in the practice and over the telephone for the general practice population. Assess, diagnose, plan, implement, and evaluate treatment / interventions and care for patients presenting with undifferentiated diagnosis.
Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly. Assess, diagnose, plan, implement and evaluate interventions / treatments for patients with complex needs. Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition, as appropriate. Diagnose both acute and chronic conditions, integrating both drug and and non-drug based treatment methods into management plan.
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, and national and practice protocols, and within scope of practice. Work with patients to support compliance with the adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication, side effects, and interactions. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent, or emergency situations, including the initiation of effective emergency care.
Provide safe, evidence-based, cost effective, individualised, patient care. Offer a holistic service to patients and their families, developing, where appropriate, an ongoing plan of care / support with an emphasis on prevention and self-care. Refer patients directly to other services / agencies, using national and local guidelines. Work with nursing and medical colleagues to ensure national service frameworks are being delivered.
Work with multi-disciplinary team within the practice and across the wider NHS network to promote integrated and seamless pathways of care. Review communications and letters, and action them within the scope of practice. Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery. Help develop and set up new patient services and participate in initiatives to improve existing patient services.
Teaching and Mentoring Role Promote a learning environment for patients, nurses, and other health professionals. Contribute to the planning and implementation of the teaching for existing practice staff and nurses. Professional Role Promote evidence-based practices through the use of the latest research-based guidelines and the development of practice-based guidelines / protocols. Monitor the effectiveness of your own clinical practice through quality assurance strategies, such as the use of audit, peer review, and appraisal.
Maintain the professional register and revalidation. Participate in continuing professional development opportunities to ensure that up-to-date, evidence-based knowledge and competence in all aspects of the role are maintained. Work within the latest NMC / HCPC code of professional conduct. Record accurate consultation data in patients’ records in accordance with latest NMC / HCPC guidance and other pertinent standards.
Keep up-to-date with pertinent health related policy and work with practice team to consider the impact and strategies for implementation. Work collaboratively with colleagues within and external to the practice. Demonstrate leadership. Delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence.
Manage on the delivery of specifically identified services or projects as agreed with the practice management team / clinical lead. Discuss, highlight, and work with the team, to create opportunities to improve patient care. Managerial Role Participate in clinical supervision for the clinical team and act as clinical advisor to other members of the primary healthcare team. Encourage and develop teamwork within the practice.
Participate in multi-disciplinary protocol and practice policy development. Identify appropriate opportunities to delegate both clinical and administrative tasks to more junior staff. Help the practice operate in a cost effective manner. Participate in practice and clinical meetings, reporting progress as required.

Knowledge, Skills, and Experience Be registered as a General Nurse of Paramedic equipment Minimum five years post registration experience Minimum four years experience in primary care BSc in appropriate practice domain Demonstrate evidence of study at master’s level / master’s degree Clinical skills training / qualification and experience Non-Medical Prescribing qualification Knowledge of chronic disease management Advanced clinical practice skills, management of patients with long term conditions Management of patients with complex needs Teaching and mentorship in a clinical settin
