Description:
An exciting opportunity to be part of a new comprehensive front door streaming service has arisen for an enthusiastic Advanced Nurse or Paramedic Practitioner to join our CQC rated ‘Outstanding’ Trust, within our Urgent Care Service.
The successful candidate/s will undertake safe and efficient front door streaming service for patients attending ED, ensuring patients who are suitable for primary healthcare are diverted to o ur Primary Care Centre which is based on-site at the Royal Berkshire Hospital .
Our current vacanc ies are for Practitioner s who can cover weekdays 6 .30 pm until 10.30pm and Saturday, Sunday s and Bank Holidays 10am until 10.30pm .

Working closely with our ED colleagues , y ou will be part of a forward-thinking team of GPs, Practitioners, Nurses and Healthcare A ssistants , providing high quality out of hours care to the local population .
Perform streaming safely and efficiently, assigning patients to the correct department and under the correct priority level.
T aking a brief history and performing basic observations if appropriate .
Request and interpret tests appropriate to patient’s needs.
Follow diagnostic and referral pathways .
Effectively liaising with a range of inter-professional teams including acute colleagues and primary care colleagues.

Responsibilities:
Registered Nurse/Paramedic
Post Registration Advanced Clinical Practice or have completed 2 or more clinical skills modules
Non-Medical Prescribing (NMP) (desirable)
Minor illness experience
Able to work well under pressure
