Description:

Responsibilities and duties will include, but may not be limited to the following To provide assessment, diagnosis and treatment, of both acute and chronic conditions, acting as an autonomous clinician with the support and guidance from the GPs. This will include initiating appropriate treatment and /or referral, including admission to hospital if required. It would be desirable to have the skills to assess children as well as adults. To act as practice liaison to our two residential care homes for adults with learning disabilities as well as arranging regular weekly visits to assess both acute and chronic conditions and undertake annual LD reviews.
To proactively manage our housebound and frail patients with input from our practice care co-ordinator, to identify any early clinical need and reduce deterioration and admission. This will include visiting them at home and assessing any acute or chronic conditions. This may include working with outside agencies to ensure appropriate care. To undertake a share of daily home visits if needed.
To maintain adherence and familiarity with locally agreed healthcare pathways and signpost appropriately to other services. Ability to support GP administrative workload with suitable training and support Support and lead the practice nursing team including mentoring as appropriate. To proficiently use the clinical IT system for safe and thorough documentation of patient care as well as extract information for clinical governance. To work collaboratively with other members of the health care team such as the District Nursing Team, Mental Health Services, staff in Care Homes, Ambulance Service, and Secondary Care, etc.
To engage in self-appraisal and ensure practice is within good clinical governance and based on sound up-to- date clinical evidence. Where appropriate, to identify and assess any risks associated with patient care or to oneself and communicate these risks to the appropriate clinical manager. To ensure effective time management and prioritisation of workload. Ability to work within own scope of practice and understanding when to refer to GPs To participate and contribute to clinical meetings.
To work within the requirements of Data Protection legislation as it applies to patient records. To maintain current professional registration and indemnity
