Advanced Nurse Practitioner

The Nurse Practitioner/Clinical Practitioner will work autonomously within their professional scope of practice providing acute care for patients via face to face, telephone and online consultations and when required, home visits (these are very occasional). The ANP/ACP will need to provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care, making necessary referrals for investigations in the appropriate manner. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. They will demonstrate critical thinking in the clinical decision-making process and will work collaboratively with the practice team to meet the needs of patients and support the delivery of policies and procedures
