You will be working closely with the multidisciplinary team and other stakeholders within the 50 bed facility. This position is responsible for the provision of person-centered, individual care to residents. DUTIES:

  • Promote the physical and emotional wellbeing of residents
  • Ensure person-centred support and support plans are at the forefront of everything you do
  • Partnering with residents to be involved in the meaningful activities of their life
  • Coordinate with other staff to achieve outcomes and liaise directly with nursing staff regarding any specific health concerns
  • Support with everyday activities such as;
  • Eating
  • Personal hygiene tasks (such as bathing, dressing, personal care)
  • Mobility
  • Household tasks
  • Leisure activities
