Description:

A new opportunity has been developed for a Duty Coordinator to work within the Urgent Community Response team supporting the Allied Health Professionals across the Virtual Ward and Urgent Care pathways.
The Virtual Ward forms part of the Urgent Community Response (UCR) team. We are a 7-day service that supports acutely unwell patients who would otherwise be in a hospital bed, supporting both early discharge from hospital and admission avoidance. The team responds to the needs of patients in the place they call home. The allied health professions within the team are Occupational Therapy, Physiotherapy, Dietetics and Speech and Language Therapy.
We require a dynamic individual to play a vital role in supporting the Allied Health Professional team. This will involve the triage and prioritisation of referrals, completing wellbeing calls and providing support with urgent therapy visits for assessment, equipment provision and rehabilitation of patients.
You should be confident, proactive and have excellent communication skills with a passion for patient care, with drive, enthusiasm, and talent to develop services to support patients to remain at home where possible.
The post holder will have previous experience in a clinical health setting. You will be part of a highly supportive team using an inter-professional model to support a broad range of patients. You will be supported with continuing professional development and clinical supervision.

AS A BAND 4 DUTY CO-ORDINATOR YOU WILL:

Work as part of a team but also be confident in working autonomously
Lead in therapy referral triage
Support the co-ordination of patient management by allocating visits and managing effective admission / discharge services and identifying and completing appropriate onward referrals
Work flexibly to visit patients in their own homes, carrying out basic assessments and treatment plans, and supporting therapists with their patient caseload as required.
The ability to travel around the county in is essential
