Description:

To comprehensively assess patients, gaining information about their physical health, mental health, social issues as well as their alcohol history to format a plan of care based on the patients individual needs.
With the support & guidance of senior nurses, advise & support emergency department staff on admission criteria for patients with alcohol issues attending the department.
To liaise with & build strong professional relationships with other healthcare professionals both within the hospital & external agencies, to meet the needs of the patient.
To develop therapeutic relationships with clients.
To screen patients to identify those at risk from alcohol related health and social problems (hazardous drinkers) & those whose health is being damaged by alcohol (harmful drinkers) & undertake appropriate interventions or referrals.
To undertake individual case management to provide immediate & on-going support to those patients who are unsuitable or unwilling to engage in community services.
To attend multidisciplinary meetings to discuss referrals, frequent attenders or patients in treatment.
To make accurate assessments of the risk patients pose to themselves and others & take necessary action within Trust safeguarding guidelines.
To educate patients & their families on alcohol related harms & deliver brief advice.
To provide training to Trust staff on screening and brief interventions, and to participate in the development and delivery of education programmes.
The band 5 Alcohol Care Team Nurse will be an integral member of the Alcohol Care Team which provides care and treatment for patients with alcohol misuse issues. This nurse will assess patients who misuse alcohol and advise Trust staff on the care and management of acute alcohol related issues and care plans. They will participate in the development and delivery of education programmes for Trust staff in the speciality of alcohol misuse. The band 5 nurse will be responsible for supporting a caseload of individual patients who are not engaging in the community which results in hospital admission and frequently attend Trust services. The band 5 alcohol care team nurse will be willing to be flexible to meet the needs of this new team and must demonstrate knowledge, expertise and passion in working within the speciality of alcohol misuse. The job role will include cross site working but will predominantly be based on the North Tees site. The working hours are Monday to Sunday day time hour’s dependent on the service need..
