Description:
An exciting opportunity has arisen for an Ambulatory Care Coordinator within the Clinical Haematology department at Sheffield Teaching Hospitals NHS Foundation Trust.
THE SUCCESSFUL CANDIDATE WILL:
Co-ordinate the scheduling of treatment on the ACU
Co-ordinate the scheduling of support treatments for Haematology patients on the ACU
Co-ordinate the flow of patients through the ACU maximising the effective use of staff and chair resources
Liaise with other teams, service and departments to facilitate effective scheduling and flow of patients.
Identify actual and potential problems with scheduling of treatments, such as breaches in capacity for delivery, and take appropriate action to prevent or address issues identified.
Provide data and reports to support scheduling and workflow in ACU
Participate in Service Improvement initiatives carried out relating to Haematology treatment delivery.
Co-ordinate the scheduling of treatment through the Ambulatory Care Unit liaising with pharmacy, out-patient, medical and nursing services.
Have a good understanding of the systems and processes that underpin the Haematology pathway to be able to anticipate and take appropriate action when unplanned challenges to Haematology service delivery occur.
On a daily basis formulate the Haematology appointment list. Identify concerns regarding scheduling of treatment appointments and take action to resolve them in conjunction with the ACU nurse in charge and pharmacy services.

The Ambulatory Care Unit is based within the Haematology Day Unit which provides care and treatment across the South Yorkshire Region.
This Service is a new and exciting development in Haematology. The purpose of the unit is to develop and provide an efficient effective and caring ambulatory service to patients from across the South Yorkshire region who meet the ambulatory criteria.
