Description:

An exciting opportunity has arisen for an enthusiastic and dynamic individual to apply for the role of CNS across Carmarthenshire and based at Glangwili General Hospital. As a member of the team you will work with the Anticoagulation CNS team, Consultants and the Multidisciplinary Team. Excellent communication and time management skills are essential to work collaboratively with patients, the public and professional staff across the spectrum of health and social care, to deliver and continue to develop a high standard of care for patients receiving anticoagulant therapy aiming to promote health and prevent disease.

COMMUNITY FACILITIES, INCLUDING:

48 General Practices (GP surgeries), 49 Dental Practices, 98 Community Pharmacies (chemists), 44 General Ophthalmic Practices (including eye health and low vision services), 38 sites providing mental health and learning disability services, care within your own homes.
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

Responsibilities:

To promote a high standard of patient care in all aspects of anticoagulation nursing.
To accept new referrals for anticoagulation, conduct assessment interviews and provide on-going education of patients.
To run anticoagulation monitoring clinics and supervise the practice of support workers.
To interpret results and determine anticoagulation dosing using computer assisted decision-making software.
To assess and initiate treatment for patients referred from their GP and A&E on an ambulatory DVT Pathway.
To provide anticoagulation / thromboprophylaxis teaching on medical induction & nursing preceptorship days.
To take the lead on anticoagulation audit, and to assist with the audit of thromboprophylaxis.
Manage the delivery of nurse led anticoagulant services in the Health Board with clinical support of the Consultant Haematologists.
Accept direct referrals from primary and secondary health care professionals for patients needing anticoagulation therapy, avoiding the need for hospital admission and promoting early discharge.
Deliver nurse-led care following agreed protocols for the initiation and maintenance of anticoagulation therapy for outpatients with atrial fibrillation. Through nurse-led clinics, utilize expert knowledge and clinical skills . Analyze, interpret, act on and communicate results sensitively to the patient and referral source. Analyze, co-ordinate, organize and monitor the treatment of patients receiving anticoagulation therapy, following agreed protocols, including dose adjustments according to blood test results and managing emergency treatment for patients at risk of thrombosis/bleeding due to under/over anticoagulation.
