UChicago Medicine AdventHealth Bolingbrook, GlenOaks, Hinsdale and La Grange hospitals are affiliated with UChicago Medicine, one of the top academic medical centers in the country. We are also part of AdventHealth, a unified health system with over 50 hospital campuses and hundreds of care sites in diverse markets throughout 9 states.
Responsibilities:
UChicago Medicine AdventHealth is hiring for their Community Palliative Care program. This is a growing program which is so needed in our communities. The program serves our homebound patients, giving them access to palliative care in the comfort of their homes.
- Experienced APP’s preferred, but new grads welcome to apply
- Weekly Monday - Friday schedule
- No call requirement
- Patient visits in assigned territory
- Outpatient palliative care in patient’s homes
- Work collaboratively with Social Workers and House Calls Physicians & APP’s

Qualifications:
EDUCATION AND EXPERIENCE REQUIRED:
- Master’s degree of Nursing or Physician Assistant Studies required.
