Description:
An exciting opportunity has become available for a dynamic Mental Health professional to join our expanding Primary Care Mental Health team in Neath Port Talbot.
The Local Primary Mental Health Support Service (LPMHSS) has been established to meet the requirements of Part 1 of the Mental Health (Wales) Measure (2010). Our service offers, assessment, signposting and brief interventions to people with mild to moderate, common mental health disorders.
The service also aims to support the wider Primary Care team through advice, education and support, as well as offering information and advice to carers.
The main functions of the post will be to screen, assess, signpost and provide brief follow-up interventions to primary care referrals within an agreed GP community network within Neath Port Talbot . The post holder will have extensive experience of undertaking assessments in a mental health setting and will have knowledge and experience of working within the community. New practices have been introduced due to the COVID-19 pandemic and assessments are primarily carried out via telephone or virtually, therefore computer literacy is key.
The post holder will work within a Primary Care team that currently comprises of Qualified Mental Health Professionals, an Occupational Therapists and small Support Worker Team.
