Description:

An exciting opportunity is available for an Assistant Practitioner to work as part of the team to deliver Community Cardiology service and Cardiac Rehabilitation in North/Mid Powys.
We are willing to offer this post as an Annex 21 development opportunity, to applicants with less experience/knowledge, subject to existing qualifications.
Powys Teaching Health Boards’ cardiology service provides provision for the residents of Powys by developing a direct access, one-stop shop community cardiology service and comprehensive cardiac rehabilitation programme provided by a multi-disciplinary team consisting of a broad range of specialist staff.
A large part of this exciting role will be supporting the provision of Cardiac Rehabilitation in North and Mid Powys. Phase 3 Cardiac Rehabilitation is provided in Community settings across Powys, offering high quality evidence based rehabilitation to patients with a cardiac diagnosis. The post holder will also be responsible for their own case load in setting up Event Monitoring devices for patients, all training will be provided. The post holder will be responsible for collecting and entering National Data for the Cardiac Rehabilitation Service (NACR) and completing onward referrals for patients who have completed Cardiac Rehabilitation.
As an Assistant Practitioner the post holder is expected to manage their own work and caseload independently, carrying out tasks delegated by a Registered Practitioner with appropriate supervision in place. Understanding evidence-based practice and providing care in accordance with current evidence, the post holder will take responsibility for providing person-centred care in accordance with the organisation’s policy and procedures.
Within the role the post holder will b e responsible for some elements of patient assessment, which is done in conjunction with the registrant, implement care programmes. Report regularly to the Registered Practitioner, recognising their own limitations and seeking advice if in doubt.
