Description:

Merseycare are looking for two enthusiastic, hardworking Band 4 Assistant Practitioners to join their innovative and dynamic Equipment Advisory Service (EAS), working to support Liverpool and Sefton Community Equipment Services. The positions could be suitable for an existing Assistant Practitioner or a Health Care Assistant with the right level of experience.
Your key role would be to support the qualified clinical equipment advisers in the assessment of adults and children for safe and suitable community equipment provision, providing and sharing your knowledge and skills in relation to assessment and provision of community equipment.
Ongoing training in relation to assessment skills and equipment available will be provided.
The service works collaboratively with many other services around Liverpool and are always open to new ways of working together to ensure safe and effective equipment is provided in a timely manner.

To give advice on appropriate community equipment within the patient home environment.
To Support the prescribing of equipment in the community, to prevent hospital admissions and promote safe discharges from secondary care.
To assist the Clinical lead with training and education on safe and appropriate prescribing of equipment in the community setting.
To manage own caseload and work autonomously.
Provide clinical support and knowledge to support staff
Undertake risk assessments on defined patient in line with relevant guidelines

The post holder will have continuing responsibility for the management of a defined caseload under the supervision of a qualified practitioner. The primary function of the role is to support the responsive services of the Equipment Advisory Service. This will be done via a holistic assessment of care needs, joint assessments with qualified practitioners and supporting with the running of the EAS training programmes. The post holder will support qualified practitioners with community equipment reviews of the catalogue and ensuring equipment is issued and recorded effectively.
