Assistant Practitioner for Integrated Transfer of Care (ITOC)

The Assistant Practitioner will provide rehabilitation and support within the ITOC team, in a variety of settings. This role requires a motivated and dynamic person to work within our established service, and it is essential that you have experience in working as part of a multidisciplinary team.
You will have experience of treating patients with complex disability/orthopaedic conditions/delirium dementia and falls. Experience of being flexible and working in a variety of unplanned care settings i.e. A&E, with Frail patients in the community and Discharge support in the Acute Hospital and in our Recovery hub.
You must have good communication skills and flexible approach. A formal Assistant Practitioner qualification is desirable, but at least willingness to work towards this is essential.
