TITLE: Assisted Living Nurse

Responsibilities:
- Under the direction of the CNS, assure residents have up to date care plans and service plans.
- Monitors and maintains resident charts and clinical progress notes.
- Communicates to physicians, other personnel (county caseworkers), and designated representatives (per resident preference) changes in resident’s needs or conditions.
- Facilitates coordination of resident services with other service providers within the current service limitations as described in the UDALSA.
- Ensures and participates in all aspects of medication management services for applicable residents.
- Ensures all physicians orders are signed and implemented timely.
- Completes assessments per regulations and within scope of practice.
- Assist with admissions as directed by CNS and/or Housing Manager.
- Under the direction of the CNS assure that delegated nursing tasks are followed properly and discuss any needed corrections, errors, etc., with staff.
- Works as a team player with all members of the resident’s care team.
- Ensures safe practices of medical equipment being used in the building.
- Ensures that staff clearly understand and follow all resident care procedures.
- Shares resident and family concerns with pertinent staff if appropriate.
- Assists in the development of policies and procedures and implementation of them.
- Attends routine staff meetings and trainings.
- Is flexible and willing to expand and adapt to changes in the industry.
- Promotes a positive, team approach and a healthy work environment.
- Ability to deal tactfully and use good communication and customer service skills with staff, residents, families, and professional encounters.
- Is available for on-call medical questions and/or emergencies via phone. Occasional need to assist in house when on call.
- Ensure compliance with current regulations and rules.
- Obtain required training upon hire and annually thereafter to meet regulatory requirements.
- Must obtain 8 hours Dementia Training within 80 hours of start of employment.
