POSITION SUMMARY
The Couchiching Ontario Health Team, approved in the first wave of OHTs in Ontario and recently accelerated as one of twelve OHTs at a high level of maturity to move forward with specific initiatives, requires ongoing clinically informed leadership to meet the objectives laid out by Ontario Health in the Path Forward. The COHT Joint Executive Team (JET) on behalf of the OHT members, is seeking a clinical leader to develop and grow the engagement of primary health care providers across all disciplines (Family Physicians, Nurse Practitioners, Midwives and others) and Specialist Physicians.
As part of this process, COHT Associate Lead Primary Care will work effectively with the COHT Primary Care Network, COHT Primary Care Leadership Council, and the COHT Lead Primary Care Physician, the local Communities of Practice, and with peers and other clinicians, to clinically represent the consensus of the whole, during the planning and implementation of initiatives that improve integrated care, providing leadership for mutually agreed upon accountabilities.
The COHT Associate Lead Primary Care will be supported by the OHT Support Team.
POSITION SUMMARY:
The COHT Associate Lead Primary Care will engage with primary health care clinicians and physician specialists to work through a collaborative process and agree to:
- Act as the Chair of the Primary Care Network Leadership Council (PCN-LC), either in the absence of the Chair or as requested by the Chair, and to carry out additional leadership duties relating to the functioning, responsibilities and effectiveness of the PCN-LC.
- Assist and advise the Chair in providing independent, effective leadership to the PCN-LC
- Regularly attend at Primary Care Leadership Council Meetings and Primary Care Network Summit Events
- Ensure fulsome engagement with COHT Patient Family & Advisory Council
QUALIFICATIONS
- Education
- A practising primary care provider holding a valid certificate of registration and in good standing with their respective college
- Formal leadership training, change management and experience in academic detailing or mentoring considered an asset
- Experience
- Actively practising within the COHT region
- Experience applying a health equity lens to projects/initiatives aligned with the quadruple aim (population health, patient experience, per capita cost, provider experience) preferred
- Knowledge
- A clear understanding of the planning and implementation of large-scale initiatives across all sectors of the health care system, especially those that may be impacted by or may impact primary care
- Knowledge of provincial health care structures, processes, funding mechanisms and current priorities is strongly preferred
- Understanding of clinical leadership in health system design, planning and implementation, with specific expertise in multi-sector and intersectoral engagement.

Responsibilities:
KEY RESPONSIBILITIES
Integrated System Design
- Provide leadership and support to the design and implementation of the Couchiching Ontario Health Team in alignment with the purpose of the COHT
- Follow guidance from the COHT Collaboration Table, actively collaborate with the COHT Primary Care and physician specialist representatives, the Primary Care Transformation Lead for Ontario Health, the Ontario Health Central Clinical Leads, and all COHT Collaboration Table representatives from all sectors/clusters (Patients/Clients/Care Partners, Mental Health & Addictions, Hospital, Community Support Services, Primary Care, Home & Community Care, EMS, Long-Term Care, Indigenous Health, Public Health, City/Municipal partners etc).
Operational Clinical Leadership
- Provide leadership, represent the consensus of health care providers for the full COHT region, and collaborate effectively..
- Collaborate with the COHT in the development and implementation of priority initiatives as identified and approved by OH and operationalized by COHT JET
- Work with members and member organizations to co-develop new clinical population pathways and work to consistently measure and improve existing clinical pathways with a population health approach.
- Support clinical change management and communication initiatives to inform OHT interested parties and the broader community as clinical population pathways are implemented.
Clinical Leadership Excellence
The Clinical Lead shall demonstrate excellence in their servant leadership role and consistently reflect and strive to live out the purpose of the COHT. This shall include but is not limited to:
- Working collaboratively with all members of the care team (patients/clients/care partners, primary care, public health, home and community care, mental health and addictions, community and support services, long-term care, emergency medical services, acute care, and city/municipal partners) to improve population and individual patient health outcomes, patient/client/care partner experiences, provider experiences, and value for per capita cost
- Supporting the development of COHT committees and ensuring clinical representation on those committees, as appropriate, via active participation and/or coordinating physician/clinician involvement and engagement
- Consistently providing an inclusive and holistic perspective for all clinicians and being invested in ongoing engagement initiatives to strengthen the COHT and the COHT PCN.
- Supporting clinical education and developing a clinical culture of continuous quality improvement
- Investing in understanding the barriers to change management and consistently engaging with all members of the health sector to rapidly develop and iterate solutions
- Support COHT local engagement strategies, change management activities, performance measurement, physician education, primary care capacity and other opportunities to promote a continuous quality improvement culture in primary care and across sectors in the region.
